Please go to this webpage and complete the survey as an MS Word or Google document.
(You can just copy and paste into a word processing document.) When you are done, please attach the file, plus a photo of yourself (preferably within the body of the survey) and e-mail it to Les. (My e-mail is on the course outline).
Remember to include a subject in your message so the e-mail doesn't get categorized as spam. Also, indicate your class name in the subject.
